JOB TITLE: Re-entry Support/Reintegration Support Specialist
Openings: 1 Fulltime & 1 Part-time
Position Description
The Re-Entry Support Specialist is responsible for working with individual and partners inside correctional facilities, specifically, New Jersey Department of Corrections as well as in the community.
The Re-Entry Support Specialist will support individual service plans, evaluate service options, make necessary referrals and manage caseload to gauge process.
The Re-Entry Support Specialist will provide guidance and support to HBBKV’s returning citizens that are experiencing barriers post incarceration. This includes assistance with reunification of families, administering needs assessments, reviewing reentry goals and customizing goals to fit individual needs.
The Re-Entry Support Specialist will assist returning citizens secure fair non-discriminatory housing (transitional & permanent), access wrap around services in communities, conduct wellness check-ins, and other Support needs.
The Re-Entry Support Specialist will work closely with Executive Director and Case Manager to ensure program goals are met.
Essential Job Functions
The following duties are mandatory requirements of the job:
· Works with individuals pre and post release to facilitate successful reintegration into the community.
· Provides referrals to individuals for community base resources.
· Review reentry plans to develop realistic goals and personal action plans to ensure successful reintegration and addressing the five (5) keys to reentry well-being: healthy thinking patterns, meaningful work trajectories, effective coping strategies, positive social engagement, and positive relationships.
· Assist program participants with obtaining and completing forms and documents necessary for transition (i.e. transcripts, birth certificates, state identification, social security cards) and navigate challenges that having a criminal background will present (i.e. employment, housing, transportation).
· Serve as a liaison between program participants, community organizations (i.e. transitional living facilities, parole officer) and family to ensure networks of support are communicating. Coordination will provide the best opportunity for success and lower the chance of recidivism.
· Attend relevant events and activities to represent the company and build strong relationships with key community members
· Escalate complaints made by returning citizens and commences grievance process.
· Serves as the main liaison between Habakkuk Visions and other Re-Entry organizations
Other Responsibilities
The following duties are to be performed as needed:
· Attend data driven and clinical supervision, as well as departmental, divisional and staff meetings.
· Filing, faxing, copying, scanning, data entry and general support
· Ensure files are up to date and accurate
· Draft and send out correspondence as needed
· Assist with coordinating trainings/events
· Flexibility required, must be willing to flex schedule to work evenings and/or weekends on an as needed basis
· Other duties and projects, as assigned
Education and Certification
Associates degree in social work, public relations, business management and economics or related field preferred. Minimum Education requirements are: High School Diploma or G.E.D
Special Skills and Knowledge
In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:
· Demonstrated experience with program management, supervision and reporting
· Database management and reporting generation (experience with COGNOS, EHR reporting and/or other government reporting software)
· Strong communication skills (both verbal and written)
· Strong research and problem-solving skills
· Proficiency in all Microsoft applications
· Excellent organizational skills
· Ability to work independently and manage time effectively
· Creative skills and make-it-work attitude
· At least two years of work experience in public relations or a related field
· Great interpersonal skills and ability to work well as part of a team
· Ability to work well under pressure and manage sensitive or controversial subjects with tact, kindness, and professionalism
